BEFORE YOUR VISIT
Before your appointment at the Ann Arbor Center for Developmental and Behavioral Pediatrics, please complete the appropriate form below. Please bring the completed form with you to your appointment.
For Dr. Rick Solomon: Medical History Form
For Dr. Mark Bowers: Child Background Questionnaire
Here are some other helpful things you should know before your visit to the Ann Arbor Center for Developmental and Behavioral Pediatrics.
- We do not bill insurance companies. Families receive coded receipts to submit for reimbursement.
- We make the experience safe and fun for the children. Our exam rooms have lots of toys and no shots are given.
- If it is necessary to cancel your appointment, we ask that you give us at least two business days notice. This courtesy allows other patients who are waiting for an appointment to use this time slot. For cancellations after that time, there will be a $50 charge. For no call/no show appointments, there will be a charge equal to half the cost of the visit.
- Present IEPs and reports from past evaluations can be helpful. If you have this documentation available, please bring it with you to your child's appointment.
For Dr. Solomon:
- New visits last two hours, and return visits last one hour.
- Visits include history taking, physical exam and developmental evaluation (usually play based and observational).
- Reports are sent to the family and primary physician (unless requested otherwise by the parent).
- Audiotaping of recommendations is frequently provided.
For Dr. Bowers:
- New visits last two hours, return visits last one hour, and psychological testing lasts between one and four hours.
- Visits include history taking, review of relevant records and documentation, and developmental evaluation (usually play based and observational).
- Reports are sent to the family and primary physician (unless requested otherwise by the parent).
- Printed copies of recommendations are frequently provided.
- Caregivers receive a testing report, including recommendations, at the conclusion of a formal psychological/intellectual evaluation with the child.
